This 6-part kit uses the same 4-up quadrant employee sheet as the larger quadrant kits but drops the second state/local employer copy, making it a fit for filers who only need one employer copy of each type. Matching double-window moisture seal envelopes are included, so the mailing side is already covered.

The three standard 2-up forms cover the employer’s side: Copy A (red scannable) is batch-mailed to the SSA with your W-3, Copy D is the employer’s records copy, and Copy 1 is the employer’s state, city, or local copy. The employee’s four copies — Copy B (federal), Copy 2 (state/city/local), Copy C (record), and a second Copy 2 (state/city/local) — are printed together on one 4-up quadrant sheet per employee.

Employee copies arrive pre-grouped by quadrant, so there’s no collating step — print each sheet type as a batch, then fold, insert, and mail the employee sheet directly using the included moisture seal envelopes, sealed with a moistener or damp sponge rather than a peel-and-press strip. Confirm your software supports the 4-up quadrant layout before you print, since it differs from the horizontal 4-up arrangement used in other kits.
