This 6-part set pairs standard 2-up employer forms with the 4-up quadrant employee sheet, but doesn’t include envelopes — a fit for filers who want to order matching envelopes separately, choose a different seal type, or already have compatible envelopes on hand.

The three 2-up employer forms cover Copy A (red scannable, batch-mailed to the SSA with your W-3), Copy D (the employer’s records copy), and Copy 1 (the employer’s state, city, or local copy). The employee’s four copies — Copy B (federal), Copy 2 (state/city/local), Copy C (record), and a second Copy 2 (state/city/local) — print together on one 4-up quadrant sheet per employee instead of four separate pages.

The employee copies are pre-grouped by quadrant, so no collating is needed: print each sheet type as a batch, then fold, insert, and mail the employee sheet once you’ve chosen and ordered a compatible envelope. Check your software for compatibility with the 4-up quadrant layout before printing, since it’s a different form arrangement than the horizontal 4-up sheets used elsewhere in the lineup. Because envelopes aren’t bundled in, this set also works well if you’re restocking forms but already have a supply of matching double-window envelopes on the shelf from a previous order.
