4up Quadrant W-2 Form 6-part Kit – (with Self Seal Envelopes)

A 6-part kit built around one quadrant sheet

This preprinted W-2 kit uses a 4-up quadrant layout on the employee side, so Copy B, Copy 2, Copy C and a second Copy 2 print together on one sheet in the order they’ll be folded. Double-window self seal envelopes are included, matched to the folded sheet size.

Employer copies — Copy A, Copy D and Copy 1 — stay on the standard 2-up layout for filing and internal records. Because the employee copies arrive preassembled on the quadrant sheet, there’s no separate collating step: print each copy in a batch, fold, insert into the included envelope, and mail. Order a minimum of 25 and in multiples of 25 after that, counting by the number of forms you need rather than the number of sheets.
