This 8-part set gives you a preprinted, scannable Copy A plus blank stock for every other copy, split between 2-up employer sheets and 4-up employee sheets. It’s built for filers who print in volume and want the employee instructions preprinted on the sheet rather than handled separately.

The 2-up employer sheets — for Copy D and Copy 1 — have no side perforation. The 4-up employee sheets carry the employee instructions on the back and cover Copy B, Copy 2, Copy C, and Copy 2. Print employee copies as a batch, then fold and mail them; batch-mail the employer copies to the SSA and your state. Confirm your software supports this layout before printing.
