This 6-part kit splits the work between two form types: standard 2-up sheets for the employer’s own records and a horizontal 4-up sheet that condenses all four employee copies onto one page per person, paired with self seal envelopes for mailing. The horizontal orientation is what separates this kit from the quadrant-style 4-up kits, where the four copies stack rather than run side by side.

The three standard 2-up forms — Copy A (red scannable), Copy D, and Copy 1 — stay with the employer: Copy A gets batch-mailed to the SSA with your W-3, Copy D is your records copy, and Copy 1 is the state, city, or local employer copy. The employee’s four copies — Copy B (federal), Copy 2 (state/city/local), Copy C (record), and a second Copy 2 (state/city/local) — print together on the horizontal 4-up sheet, one sheet per employee.

Because each employee’s copies are already grouped on one horizontal sheet, there’s no collating step — print each sheet type in a batch, then fold, insert, and mail the employee sheets directly using the included self seal envelopes, which close with a peel-and-press strip instead of moistening. Check your payroll software for compatibility with the horizontal 4-up layout before you print, since not every W-2 program supports this exact form arrangement.
