Form 1042-S Foreign Person’s U.S. Source Income – Recipient State Copy D

Copy D of Form 1042-S goes to the recipient for submission alongside their state tax return, reporting U.S. source income paid to a foreign person that was subject to withholding. Withholding agents include this copy in a recipient’s full 1042-S packet whenever the recipient also has a state filing obligation.
