4up Blank W-2 Form 6-part Set (with Instructions)

This 6-part set is aimed at businesses printing their own W-2 data who still want employees to receive filing guidance. A preprinted Copy A handles federal submission, while blank 2-up and 4-up sheets carry the rest of the set through your payroll software.

The employer side prints onto blank 2-up sheets for Copy D and Copy 1. The employee side uses a blank 4-up quadrant sheet for Copy B, Copy C and two Copy 2 forms, with current tax year instructions preprinted on the reverse. Print employee copies as a batch, fold, and mail; mail employer copies together to the SSA and the relevant state agency. Verify your software is configured for both the 2-up blank employer layout and the 4-up quadrant blank employee layout before a full print run, and order by the number of forms needed rather than sheets.
