SKU 80558S is a double window envelope sized to pair with the 4-up quadrant W-2 blank form W24UPA. A latex self-adhesive flap closes the envelope, and both windows carry the standard security tint and cellophane glazing to keep wage data covered.

Fold the printed quadrant statement so the recipient address appears in the top window and the matching detail appears in the bottom window, then press the flap closed. Note that this self-seal version is not designed to run through inserter equipment — for machine feeding, confirm the correct envelope with a sample first.

Because this envelope is matched to a specific blank W-2 quadrant form, small differences in software print margins can shift the windows out of alignment. Ordering a free sample before your full mailing run is the simplest way to catch a mismatch before it costs you a reprint.
