3up Blank W-2 Form 6-part Set (without Instructions)

Fully blank sheets, no preprinted instructions

This 6-part set is for businesses that supply employee filing instructions separately and want the printable sheets left blank. A preprinted Copy A covers federal submission, and blank 2-up and 3-up laser sheets carry the rest of the current tax year set through your own payroll software.

The 2-up employer sheets, used for Copy D and Copy 1, have no side perforation. The 3-up employee sheet holds Copy B, Copy C and Copy 2 together, separated by a 3/4-inch side perforation, and is completely blank on the back — no instructions are printed. Print employee copies as a batch, separate along the perforation, fold and mail; mail employer copies together to the SSA and the relevant state agency. Check your software’s W-2 print settings before a full run, and order by the number of forms needed, not sheets.
